Dale is a 48-year-old small business owner with a 20-year history of severe depression, leaving him unable to work for several years. He had tried numerous antidepressants and failed a course of Electroconvulsive Therapy (ECT). He was so profoundly depressed that he rarely left home, and on many days, refused to get out of bed. His concentration was so poor that he had not driven a car in more than a year.
